Health Records
Florida Health and Immunization Requirements
A student may not attend school until all health and immunization records are complete.
All students must submit a copy of their certificate of live birth.
K5 & Older: Each child must have a "Student Physical Examination Form #3040" and "DH Form 680" (shot record).
K2, K3 & K4: Each child must have "DH Form 680" to certify their immunization.
